  • A front and back copy of Drivers License
  • A front and back copy of CPR Certification card
  • Resume and 2 professional references
  • Professional License (RN, LPN, OT, ST, PT and/or MSW)Proof of 2-step TB test results, completed in the last 30 days
  • If applicable, proof of recent flu shot
